Sakura isn't dieing before Tsunade, Sakura is supposed to surpass Tsunade, so I'm thinking she has a more power regeneration technique, and we seen the levels of madaras regeneration, and it is safe to assume Tsunade's is on that level. Also Kishi just made the perfect opening for her to activate her technique and I believe he will say something like due to her perfect chakra control, the chakra rod was ineffective, causing her to remove it and regenerate with ease
