They have the curse of hatred that in turn makes them power hungry. As Tobirama explained, the Uchiha are very emotional towards things they care for deeply; family, friends, village. If those are taken from them or put in jeopardy, then they unlock their dojutsu through special chakra released into the brain and ocular nerves.

Simply put, if they lose someone(s) they care about, the "curse" becomes active and it unlocks their powers. The more they reflect on the loss or the more they lose, the more powerful they become until they gain 3 tomoe. From that stage they either remain at that level, or progress to MS if they have felt enough pain and conflict.

(NOTE they can also train to gain their tomoe, but the sharingan is only activated once you feel the pain of loss or someone close to you being put in danger!!)

who knows how shisui got his ms but itachi got his ms when shisui died didnt yu watch the last episode and obito got his sharigan when kakashi lost his eye and he got his ms when he seen kakashi stab rin with the chidori bro
