Orihime's ability is different. It's not really healing since she rejects whatever happened, returning it to the original state. It's like reversing time.

So she can use izanagi without losing anything !!!!!!!!!!!!!!WAHAHAHAHAHAHA

maybe because it the eye he has is considered a jutsu sort of, that might be why it is not regenerating

he was brought back with the animation wrong to because it said that when you die you are re-animated just like that look at nagato and his hair for example when madara died he had one sharigan eye but plot some how managed to bring him back wit 2 rinnegan when he only had one eye when he died think back too when he left it all too obito and unplugged from the gado mazou and then when he got brong back to life no eyes and still dodging attacks I don't understand

Edo Tensei is a space/time jutsu that contradicts reality. Kabuto didn't elaborately explain it, but the specific incantation of the jutsu combined with Madara's DNA, which manifested Rinnegan at some time allowed him to bring Madara back in his prime. Madara resurrected without his eyes because he was real flesh and blood again and left his real eyes preserved in the living world with Nagato.

He resurrecting without the base sharingans he took from someone is likely explained by he being resurrected young rather than old when he implanted them into himself. Furthermore, they weren't part of his DNA, they were someone else's eyes.

Orihime has limited reiatsu as well and she has shown to not be able to resurrect others after a certain amount of time has passed.

Kubo created her shun shun rikka before Kishimoto created Izanagi btw.

Madara eye still exists so therefore I don't think it will regenerate otherwise he could create rinnegans faster than a factory by pulling his eyes out and regenerating more so no I don't think it's a plot hole =D
