We first saw Gai activating the Eight Gate here:

The red steam around him is actually blood vapour, as confirmed here:

The evaporation of blood will cause a significant drop in blood volume. Thus, Gai can only keep the Eight Gate on for a very short time before his blood runs out.

Upon opening the Death Gate, Gai experienced excruciating pain from just using the first step of Evening Elephant on Madara:

He quickly developed tolerance to the pain and managed to do successive steps of Evening Elephant.

However, the side-effects of Evening Elephant were too much - he was left gasping and panting for air.

Gai decides to give one final blow to Madara knowing that Evening Elephant won't work and releases chakra from his heart's pressure point:

If the Evening Elephant caused that much strain on Gai's body, then you shouldn't be suprised what Night Gai would do to his body.

Gai's final attack deals a devastating blow to Madara, so much so that he managed to break Madara's left ribs, but in the process of doing so he fractured the middle shaft of his lower leg bones and the distal shaft of his upper leg bone. Gai is now in no condition to walk.

The red steam has effectively turned his body into ashes:

The chakra stored in his heart's pressure point is diminishing in size:

Gai's feet, hands and even part of his face are crumbling due to the intense heat of the red steam. He has third-degree burn all over his body and at this rate, he could lose his life at any moment.

However, Naruto saved him from instantaneous death by restoring the chakra in his heart's pressure point:

Nevertheless, Gai is still in terrible conditions. I suspect Kishi will give Gai a chance to say one final farewell to his colleagues before he dies. It is very unlikely that he will survive from all of this. Even if he does (which I'm 90% certain that he won't), his life as a ninja is all over. Many of you have stated that Gai won't die but there's no confirmation of that. Gai's body is in a very feeble state right now - he was an inch away from death but Naruto has temporarily granted him life. I think he will die in the next chapter. My main point is that before you complain about all this and that, wait until something happens before you actually complain about it.

If he was going to let him die (as he should) what is the point of showing his leg break in the panel right before is heart is about to give out (when Naruto rushed to his aid). It seems unfortunately likely that Gai will suffer severe minjuries and come close to death, but survive. Which kinda sucks, but who knows, might work out.
The point of showing that is to convey the extreme stress Night Gai places on the opponent and the user. I am almost certain that Gai will die, but Kishi's the one who's writing this manga.

Tsunade is still alive. She can pretty much heal anything. Though the ball is in the air on whether he'll ever be able to use taijutsu like before again

And naruto is back with the kyuubi, I'm sure he could transfer a bit when he gets the chance

But it needs to happen now for Gai to be revived miraculously. Time is of the essence. Plus, Tsunade hasn't received the Kyuubi's chakra from Naruto yet (I believe). Naruto has to physically touch Tsunade in order to achieve that and Madara ain't sitting idle to let that happen. However, Madara is an arrogant guy so who knows what he will do...
