We done know if Kushina if she is actually dead or not. If i recall correctly the day the nine tailed fox attacked the village Naruto was born. She could possibly have died or child birth i have no idea.

Its strange she wasnt there for naruto if she was alive though unless something happened to her and she got captured etc.

kushina was the first jinchuuiken but the seal is weakend while birth. madara knew this and attacked minato and kushina. madara drained out the kyuubi und attack them (aleady 3 with naruto) after a short battle kashina und minato sealed the ying chakra of the kyuubi into naruto and the yang chakra attcked konoha but before that madara wanted to kill naruto to gain the whol kyuubi chakra and attacked him with the fox but minato and kushina protected naruto with their life
