it doesnt matter what its used for its still genjutsu and not only that since its used for manipulating why wouldnt itachi just manipulate the affected shinobi and make them kill themselves or even itachi can kill them when ka is in effect, not only that but ka renders the affect shinobi useless forever unless the user releases it which is why itachi programmed ka to stop sasuke to do just that...

As far as feats and what's been shown of him, I'd say Itachi.

Shisui is the best by hype. Obito's genjutsu was compared to Shisui's KA, so he should be close as well. By feats however, Itachi is a better genjutsu caster, but in terms of power, Madara's seems more powerful considering the fact Itachi's tsukuyomi was compared to Sasuke's, a genjutsu that failed against a immobile A when Madara's affected the same A
