This, and because Gai has a history a not remembering faces, took him a while to remember Kisame's face. As for Madara, no idea but it's pretty obvious that Madara was talking to Gai. Logic points to Kabuto.
All Uchiha are dead other than Sasuke, Madara, and Obito.

We might know that mysterious person might be related to the Uchiha or maybe that person knows a lot about the Uchiha, and by that we can tell that person will save and help Sasuke to defeat Madara.
