Kabuto is underrated because people think Edo is his crutch and he was beaten by a broken justu(which was needed because Izanagi existed). Most top level shinobi wouldn't be able to defeat Kabuto without doing it in high difficulty. His intel on top ninjas,poisons, and justu is as good, if not, better than Obito and Ororchimaru.

When Kabuto fought Itachi and Sasuke he had a major advantages. I don't consider them not killing Kabuto as an advantage so much since Itachi is an Edo and they needed him alive(not meaning they weren't willing to seriously wound him. The same could be said for Kabuto towards Sasuke).

Kabuto had the advantage of:
Them having little intel on him
Being in a cave(With DSM, his agility and speed helped him navigate the cave a lot better then Sasuke or Itachi)( Him having inorganic reincarnation in a cave where the ceiling can attack you).
Having Intel on Sasuke and Itachi.
Sage sensing( no need for eye contact or even looking in their direction)

To be honest, Susanoo and Izanami covered their asses during their fight with Kabuto. Them having EMS/MS, and it being 2 on 1 curved the advantages Kabuto had. That was the only thing keeping Kabuto from overwhelming them in that cave. He was giving two high level Uchihas a hard time. Without Izanami that fight would of most likely been Kabuto's win.

But I believe outside the cave, a fight against Kabuto would be different. That being said, I think Tsunade or any of the kages could take on DSM Kabuto. With Tsunade winning with high difficulty. Even if Tsunade or the other Kages loses, it definitely wouldn't be a stomp either way.

DSM Kabuto does beat Sage Naruto during the pain arc and MS Sasuke, and EMS sasuke before he gained PS.
