Itachi used it in Kabuto Fight and if Madara accepts his fate then He technically isn't deranged anymore but while he is in that loop they can choose to kill him or seal him.

Itachi was an edo and got fatally wounded more than once while setting it up. This is why Itachi stated it's not a jutsu to be used in real battle. For Izanami, Madara who need to be given a choice, if he accepts the choice in Izanami then the genjutsu is broken. The time in izanami is not real time, we see Kabuto go into a loop a few times while Sasuke ask if it worked. Madara knows all about how izanami works, he taught Obito it who taught Itachi who told Sasuke about it.

What is he pretending to be? Unlike Kabuto, Madara is not trying to define himself by adding other powers to himself. Madara actually calls Hashirama's power Hashirama's. He said Hashirama's wood jutsus, Senjutsus and healing abilities were all Hashiramas when he mentioned them. Also Itachi said it was ok if you wish to imitate the being you admire but you can't be that person. Kabuto was trying to be Orochimaru, Madara isn't trying to be the Sage, his name alone got the whole world to enter the war by fear.
