It's probable that they are referring to Obito vs Kakashi and the insides is Obito's corpse.

That makes no sense. The reference would be too obvious and Kishi made it very ambigious on purpose.

I think the fight is between Black zetsu who has merged with Obito, vs minato and Kakashi. Remember also that Obito still has part of his body as white zetsu.
So the insides, i think might be the zetsu fight against obito himself to retrieve his eye.

The internal battle between black zetsu and obito. Zetsu is apparently winning, meaning that hes getting more control over obito and closer to getting the rinnegan for madara.
