I already told you.. he didn't activate MS for that moment. He had it active a whole chapter before that already up until that point Sasuke used Kirin.

madara using using a susanoo without eyes is poor excuse for itachi using susanoo as madara had access to the power of chakra receivers which were used to transmit the power of rinnegan from nagato to paths
luckily for madara obito had a rinnegan with him at that time and also chakra recievers in his body and madara has shown several times he can control the chakra receivers from a distance

without sharingan itachi has finger genjutsu, some katons and average sized kunais/shurikens and i dont see those ending tsunade or katsuyu

It seems the manga is showing closer and closer of evidence that once the user unlocks Sussanoo , it doesn't matter what eye level they want to use..
