"Real" is all relative. For example, you have accepted "Tenzo" as Yamato's "Real" name, despite it being a second code name.

Kabuto was given his name at a young age by a person he considered to be his mother. You are hard-pressed to find a more "Real" name than that.

As for Sai and Yamato, their real names don't have any relevance to the story, so it is doubtful they will ever be introduced.

We might never know since both were orphans, maybe their parents didn't live for long enough to tell them their own names. And Kabuto had amnesia anyway, he wouldn't remember.
