I am not sure if has been mentioned yet, but Nagato had his eyes removed by Obito for the War, when Nagato was revived, he had his eyes, even though they were taken.

Madara may of had Rinnegan at the time of his death, but when Obito had taken them after his death, when he was reborn, he wouldn't have them as his living body had them removed. in the mangapanda scans, page 11 of 577, zetsu says he will take his rinnegan BACK, back, this means that Madara is blind due to Obito having his rinnegan, notice how he specifically says his LEFT, eye, being the rinnegan on Obito.

So madara is actually blind because his living body was without his eyes, he needed zetsu to take back an eye for madara, and possibly get the other one for him as well, as well as his mentioning of taking sasuke's eyes and admiring them.

Madara loses his eyes because of the Rinne No Tensei. Which revives the user as they were at the time of their deaths. The only reason Madara was revived young instead of old, is because his mortal body as an Edo was reformed into a younger version. Uzumaki Nagato was revived as an Edo Tensei, which means he would be revived as he died too, but his eyes was only removed after he died.
