Edit: Actually, it could be that Kabuto revived Madara as an old man and regressed his body back to youth with his experiments, and since Rinne Tensei revives you in the state your body is in and that was technically Madara's body now, he kept his youth. Since Rinne Tensei undos Edo Tensei, he lost all effects of Edo Tensei(immortality, unlimited chakra and his eyes), but kept the result of the experiments(Hashi's face and his youth) since those were administered after he was Edo'd.

Maybe because Madara was dead for a long time, at least ten years. As it seems the eyes are the first thing to decompose once the moisture has left the eye. That's maybe one reason he was brought back without any eyes

He is eyeless because his real eyes are not dead, they are still alive (technically speaking of course).

Think of it like this, edo tensei literally duplicates a person to exactly how they were when they died, (eg. sasuke with EMS & Itachi with MS makes no logical sense because that would give itachi 2 sets of eyes) madara was simply brought back to life & thus his eyes can't be duplicated like that.

If madara's eyes are not dead/gone/obliterated then how can he get new ones? He can't duplicate eyes because that's impossible, you need a sharingan in order to develop it into a rinnegan (which is what madara currently has, but elsewhere).

Madara's eyes lie with nagato & obito, they are still floating around so they can't be duplicated, his eyes need to be returned to him proper.

That eye was likely given to Obito after Obito joined Madara's side and before Madara died. I don't believe it was actually shown that he did that, but he probably did. And it makes sense that that is what happened because Madara does not have any eyes currently.

Madara's eyes that he died with were stocked. They were obviously taken from another Uchiha.

Here, Kabuto notes that having Shisuis eye could resurrect him, even though it was implanted within Danzo.

So what that would mean to me is, the eyes, regardless of whether or not they were transplanted into someone else, carries the originals DNA.

Meaning that if Obito died and Kakashi used his Sharingan for the ritual, it would bring Obito back to life.

It doesn't really explain why Nagato kept his Rinnegan upon reincarnation, but we already know Kabuto can modify Edo Tensei bodies.

Kishimoto probably made it this way so an extra pair of Rinnegan aren't just lying around...
