That's tough. Madara has that long hair, but Sasuke still seems to be drawn as if he would have those girlish features that some men have. Which isn't a bad thing because women tend to love that in a man as it makes them look prettier or something. So I'd say look-wise it's a tie

Honestly if you consider either of them "manly" you need to read/watch more manga/anime. Madara has had his share of badass moments, though, especially when he was reincarnated.

Well according to the manga Sasuke has shown more care for women than Madara ever did.

Examples:
- Apologizing to Karin
- Saving Karin from fodder ninja
- Saving Karin from a giant boar(It was a flashback that happened during the Chunin exams)
- Defending Sakura from Sound ninja(Part 1)

I guess you're right, but can you really blame Madara? He lived through nothing but war, he literally grew up fighting... War is basically in his blood, it's his only joy in his dark heart.
