I hear a lot of people say that you need to be troubled and denying reality for Izanami to work on you. This is not entirely true. Izanami works by capturing two moments with your sharingan and looping a frame of time between those moments. You can do that with your sharingan regardless of whether or not someone is denying themself or reality. Saving those who denied reality was just the original purpose of izanami, but that doesn't mean that it can't be used against other people.
It is like saying that I bought a shot gun to protect my home from intruders. Protecting my home is the original purpose, but that doesn't mean that I can't leisurely use it to go hunting every once in a while. If you're wondering how a non-troubled person would get out of it or what they would have to realize in order to escape, it could just be something like "you can't beat me".
I could imagine Madara putting Hashirama under Izanami. Hashirama might ask "What is it that you're trying to get me to realize?"
Madara would then say something like "You need to realize that I'm better than you Hashirama".
It is also possible that someone like Hashirama just simply couldn't get out of Izanami. In either case, the fact that someone is not troubled could not stop an Uchiha from doing whatever they want with their eyes.
