There are many interpretations for the 6 paths.

1. Could represent the 6 paths acquired via Rinnegan.

2. Could represent clans, as it's been mentioned that Uchiha and Senju are both paths and Uzumaki's were the 3rd.

3. Or the 6 Sage mode's.

Most likely this, sometimes it's good not to overthink things. But nice mind nonetheless.

But the Sage two sons inherited things from there father like

Senju son: He inherited the Sage's "body" — life force and physical energy

Uchiha son: He inherited the Sage's eyes, his powerful chakra and spiritual energy

At bold it says the sons inherited physical and spiritual energy from the Sage. Thus these two energies are use to absorb natural energy which makes a Sage. His Sage Mode is unknown but I believe it would have to be stronger than the Hashirama, Kabuto, and Naruto. Which I think forshadows Naruto and Sasuke fight with a someone that had the Hagoromo Sage Mode. What do you think?

Instead of sage mode i think he had mastery over all six elements, Which includes Genjutsu - Yin release.
Having mastery over all six elements and the first man to preach ninjutsu, defeating the juubi, becoming its jin, these are the only reasons i think he was called the sage of the six paths for.
