He's made use of Genjutsu in almost every fight he's been in pre EMS (Killer B, Danzo, Deidara, Orochimaru). After he got EMS, he fought Kabuto, and Ocular Genjutsu wouldn't land on him, and Obito is self explanatory.

He has, but you are going to have to be more specific, with your question.

Genjutsu isn't his preferred method of combat. He is much more comfortable with Taijutsu and Ninjutsu.

And Madara would be busy recovering for a minute or 2.
