Itachi wins, low diff. His taijutsu is incredibly fast. Even when Sasuke got weightless Lee level speed, Itachi's speed was still on another level.
Lee's speed back than was not enough to put down Neji; let alone Hyuuga Hiashi who is unimaginable leagues above from part 1 Neji. Itachi was sure faster and more experienced in taijutsu field than chuunin-genin level shinobi; Lee had like 1 year of experience in taijutsu; his tai in part 1 in base would lose to Itachi's.
Hiashi goes in for a palm thrust, Itachi catches his arm, breaks it, then kills him by punching him in the face repeatedly.
Itachi's physical strength is not that high and he can't execute such "strategy" on a taijutsu master who is knowledable on field; he is more than good to keep up and overcome Itachi in taijutsu; sharingan or not. His skillset on field allows him to see through and keep up with his attacks pattern.
