How do you get a Village symbol over your name? If you have to be Premium to get it, how do you become premium?

To join a village you go to User CP> Permission groups > And then send a request to one of villages and wait for your request to either be approved in which case you will be able to use village banner above your avatar or declined.

Given that one doesn't need to be a Premium User to possess a village banner, the link isn't needed.

Now, whether or not you get accepted into any of the 6 primary villages is up to their respective kages.
