I think he'd still win. He had decent reflexes while he was near blind. He's not that Sharingan dependent either and Sakura doesn't have speedy taijutsu so I think he'd manage fine. His handseal speed was difficult for Kakashi to make out with 3T so I think he'd be able to pull off crow clones, explosions, and Katon to catch Sakura off guard easily. His Shuriken jutsu and finger genjutsu could finish her off easily.

Itachi would win med-high dif with it, I think the yinseal being released can bypass his genjutsu, constant chakra flow. But she could possible win.
