Now at this point, I would think that the main villains of the entire Naruto manga are these three in particular:
1. Madara Uchiha: The central antagonist. The leader and founder of the Akutsutki as well as the creator of Project Tsuki No Me. Directly responsible for turning Obito evil, thus in turn the cause of Naruto's parent's deaths and the Uchiha Clan's destruction.
2. Obito Uchiha: Secondary Antagonist. Brainwashed by Madara Uchiha to be his apprentice and proxy, he is loyal to Madara and his plans despite being betrayed by him. The current Jinchuriki of the Shinju.
3. Kabuto Yakabushi: Sceondary Antagonist. Madara and Obito's subordinate and one of the main players in the Fourth Shinobi World War, he had mastered Senjutsu and created an entire Edo Tensei Army. Surpassing his former master Orochimaru, he managed to enter a Sage Mode that was virtually unstoppable, but he ultimately met his demise at the hands of none other than Itachi Uchiha.
Those three are pretty much the most evil characters of the Naruto manga wouldn't you agree? No one in the series are more evil than these guys. They are the very villains that set the premise of all of the events in Naruto.
Now I wouldn't consider Orochimaru a main villain. Afterall, all that he wanted to do is get revenge on Konoha and his former master Hiruzen Sarutobi. There have been quite a few rumors running around about Orochimaru turning bad and eating the fruit off the Shinju. Honestly, I am starting to think that people got the idea of Orochimaru still being evil from chapter 593 where he said this: "I have no interest in the war!! What I am still interested in...is that great looking body of yours Sasuke!"
Orochimaru was never really a villain as much as he was a psycho.
