That's fake, but surprisingly a good one.

All the panels on the right-side have been taken from various existing chapters and edited slightly. Madara's mouth should be open, but instead it was replaced with a closed one that was probably copy-pasted from another character. You'd be surprised how easy it is to edit a manga page with the right know-how.

The big giveaway is Naruto's eye. The tomoe look way too thin to be drawn by Kishi.

The 'kill myself' part was just a figure of speech. Why're everyone taking it seriously..I'm very well aware that this is manga thats not worth wasting my life over.
