Its supposed to be an ideal world, and people do figure out they are in a genjutsu. If dead people start showing up they are going to figure it out, Kakashi realized he was in a genjutsu when Itachi put him in Tsukuyomi.

And even if they didn't we would know with Neji and other coming back its just a genjutsu so that ending with them being under a genjutsu and not knowing about it is impossible.

Kakashi had a Sharingan to help me realize it was a genjutsu. Also since the genjutsu messes with the brain you will think it's normal for those people to be there. Your other memories will be repressed.
