According to the manga, Tsunade's nickname is 'Slug Princess'. This refers to her connection with the slugs, but it could also be a hint that she mastered an incomplete/ complete Sage Mode. Kishi portrayed the Sannins as equals. Jiraiya was the 'Toad Sage' of Konohagure, Orochimaru also had some sort of incomplete Sage mode, considering he stored his Senjutsu chakra in Anko's curse mark. [

According to Kabuto, there are three, EQUALLY FAMOUS places where the sage mode can be mastered. [

So its very likely Kishi foreshadowed Tsunade's future power- up, the Slug Sage Mode. Now you may ask, why she didn't used it against Pain or Madara? The reason is simple: When Pain attacked the village, her primary role was to save as many lives as she can, therefore she didn't fought. She tried to heal everyone instead of attacking Pain.

Against Madara she had no time to activate it. She was busy healing the others and not getting killed. As a medical niinja her main role is to heal and not to fight. But now the situation is different, considering Sakura mastered the Byakugou seal and she can heal the army. Tsunade can now unleash her full power.

Remember when Kabuto activated his Sage Mode? He used this handseal:

And when Madara activated the perfect Susanoo, Tsunade also used the same handseals, but we don't know what technique she wanted to use because Itachi defeated Kabuto and released the Edo tensei in the very same moment.
