How does a mistake like that even happen?

Amazing, amazing episode.... but just curious... how does something like that Madara Rinnegan mistake even happen?

You'd think that with how amazing they made this episode, they wouldn't overlook something like that?

In all serious... throughout the whole process, isn't there a person that REVIEWS the final episode for mistakes like that... and wouldn't it be a simple thing to fix that mistake before the episode comes out?

Anyone with knowledge of the behind of the scenes kind of thing care to fill in here? I was just curious.

Still a super awesome episode.

wow who really cares about it, most of us already know that madara has the rinnegan any way since we all read the manga, there arn't many anime only viewers since the fillers effed every one in their bung holes

I work in television animation. Yes mistakes happen all the time and I think this one might've happened because they don't make the shots in the same order that they will appear in. The animation director and producers assign shots and schedule them based on which animators they think would animate it best and if they're available. Then it goes straight to the people who clean up the drawings (the animator's drawings might be rough), and I think that it might have happened in clean-up. The cleaner probably just finished cleaning up like five Madara-rinnegan shots and that shot was unlucky enough to follow and the artist just continued drawing rinnegan because they were multitasking or daydreaming or listening to music and not paying attention to the fact that the drawings they're working on have a different scene number than the stuff they just finished.

Mistakes always happen and it's possible to revisit a shot and make fixes, but whether or not they actually do it depends on if there is time. The schedules are really super tight and I bet they just simply ran out of time. Nobody knows what other kinds of mistakes there were that did get fixed, they might've been worse.
