I thought Tsunade would have an "old lady mode" where she DGAF'd about her appearance and focused on power.

But Kishimoto's too afraid to let us see her real face, it seems.

It could be that the usage of sozo saise has aged her so much that her body is like 80, so it can't function as well as a young body. That could explain why she has to keep herself young when fighting, because the youthfull appearance transformation is not a genjutsu, but actually makes her body young inside out.

Hence all those times she regenerated she was actually aging her body at an accelerated rate.
