Sai doesn't know Sasuke and therefore doesn't trust him because without feelings to alter his point of view, he sees Sasuke for the danger he truly is to everyone. Sai knowing of Sakura's feelings for Sasuke wanted to see if she was letting it affect her, and though she voiced her denial, her smile told him she feels like he does about Sasuke. Both Sai and Sakura shared a moment of understanding back when they met, when he first noticed her fake smile before she punched him. He knows her better than she realizes because he's an expert at concealing emotions.
