The difference is quite simple.

She was in shock because she saw that Naruto had KM and that he had gained control over the 9 tails. She also didn't expect him to be taking part in the war because initially he was told not to. Hence the question mark.

Sakura already gave her reaction to when Sasuke arrived.

Sakura has a closer relationship to Naruto than Shizune. Thus, she would show more emotion. She also knows that he's been struggling his whole life with the 9 tails and has seen Naruto go 4-tails ect. Something Shizune hasn't experienced. Do you really think she wasn't happy that he saved her?

Sakura already saw Sasuke fight Kakashi with his MS techniques. No surprise.

This is a fan-made picture. How does it prove that NaruHina will happen and not NaruSaku? In the manga, it has been shown that Naruto and Sakura have worked together when they should. They have shown some real development, and they aren't some comedic pairing all the time.

While I will always favor NaruHina over NaruSaku, this photo proves nothing. Kishimoto could do anything with the pairings at this point. He's unpredictable.
