I could of sworn he killed the Kazekage in cold blood, attacked Itachi from behind, and attacked Hiruzen while in disguise

The Kazekage, and Itachi. Those were for mutual goals my friend.
Hiruzen on the other hand.. It was also for a goal, but Orochimaru did weep (in which we found out in chapter 600 something that he cared for the village) So I can surely say that Orochimaru didn't want to directly kill Hiruzen. Because remember he simply was smacking him around with the Hokages per say to teach him a lesson? Maybe Orochimaru's parents were killed by Hiruzen's orders (hence why Orochimaru went a little evil). All in all I don't think Orochimaru wanted to kill Hiruzen. If he wanted to he would have did it right then and there on the hokage's seat.
Tsunade is a fellow companion he grew up with her.. The situation is similar to Hiruzens. But we'll have to see.
or he could use Tsunade as a way to get Hashirama to submit
also Tsunade has senju DNA.....perhaps he wants to give it to Sasuke so he can gain access to the rinnegan

Of course. Everything Orochimaru does is to solely get ahead in life, and accomplish his goal(s).
so then it's really not impossible for him to kill Tsunade in cold blood.
But what would he have to gain by killing Tsunade? Leaving her alive gives him the asset to control Hashirama. Killing her achieves what goal? He wants this world to stay tact, so that he can accomplish his goals in learning every jutsu.
Killing Tsunade here, and now provides no possible furthering in his plans. Hence why Tsunade will not be killed by Orochimaru.
If he kills her, he can use edo tensei on her
if she were to remain alive, she could hinder his future plans.
Let's say for example, he found a way to defeat the weakened alliance with his power, but with Tsunade....she could rejuvenate them
Orochimaru, however, doesn't know that Sakura possesses the same ability/or at least on par.
at the end of the chapter, they ask jugo what Sasuke's true plans are, and then it cuts to Sakura's face, then to Orochimaru saying nonchalanty "not in good shape Tsunade" while she is split in half.
That hints at it being something diabolical.

Or Sakura simply could have heard what Sasuke also said. Same as which Jugo, and Sai had heard causing her to panic as a former lover had just rejoined them and she was filled with hope, and joy.. And now to see him say such cruel evil words such a "burn it down" which shows he still has hatred. She must have been panicked, and scared.
