I may be biased, but I'll at least back it up. I obviously say Kabuto. Kabuto can regenerate without the use of seals like Hashirama (albeit on a much lower level), heal others from a distance, can heal people so much it knocks them out (Kiba), can use Medical Ninjutsu in combat and has been performing Medical Ninjutsu since he was a small child.

But Hashirama isn't a medic.

Hashirama could heal himself without moving, Tsunade has come up with laws of Medical Jutsu, and Kabuto has been developing his prodigal medical skills since he was a small child.

uhhh Can't really say why? Sakura was trained by the best there is... But hasn't been shown on screen using medical ninjutsu in advanced feats. Besides figuring out the solution to a poison within minutes. o-o. Which is technically Kabuto has never shown. So you can't really base it.

But by the end it will be :

1. Tsunade/Sakura
2. Hashirama
3. Kabuto.

- Wasn't he about Sakura's age when he started....

Using Medical Jutsu as an attack requires knowledge... I don't know what you're getting at...

Kabuto was somewhere around 7 or 8 years old it seems and Orochimaru said he was already talented with Medical Ninjutsu.
