This is something that came to my mind and after some thought I had a little breakthrough. Like always read all of it before posting anything, this one is short.
I was thinking about this and had a revelation of sorts. This one is pretty short and to the point with a few scans for added validity. I hope you enjoy on this twisted I discovered about Izanami.

What we know
Izangi (Izanami's Counterpart) is a technique that allows the user to "escape his destiny", so in a case where he would normally die he can alter those events and keep himself alive. Danzo displays this in his fight with Sasuke.
Izanami (Izangi's Counterpart) is a technique that allows the user to "seal [the enemy ninjas] destiny", so in a case where the enemy is reluctant to accepting a truth, you can trap him within the genjutsu until said truth is realized. Itachi displayed this technique in his fight with Kabuto.

Question
If Izanami does infact trap said ninja in the genjutsu until he comes to grips with the truth, then who in this jutsu determines the truth. In Itachi's fight with Kabuto when Izanami was used it is pretty obvious what the truth was going to be, the fact that Kabuto was running from who he really was. But if this was used on an enemy who's truth wasnt known, then what would the truth be?
Example: Let us say that Hashirama runs into Itachi and both of them have no intel on each other, If Itachi used Izanami on Hashirama what would the outcome be? Which truth would Hashirama be forced to overcome?

Answer
I believe that Izanami is a genjutsu cast by a ninja but CONTROLLED by the enemy ninja.
Then what would the truth be if cast on someone you have no intel on? It depends completely on the ninja, they themselves control the difficulty of leaving the genjutsu. If they have no inner conflict then the genjutsu would either have very little/vague conditions for exiting it or it would be ineffective and may or may not still take the casters sharingan.
Let me use Kabuto as an example:
Durnig his fight with Itachi Itachi says alot of things to rile Kabuto up and get him angry, he then repeatedly tells him that he needs to accept himself for who he is. But why is that? Itachi isnt the type of person to talk about nonsense like that during a battle especially against someone as strong as Kabuto. Also notice that he only starts talking about this stuff after initiating Izanami.
It is because by repeatedly telling Kabuto those things he was preparing his mind to make the condition of him leaving the genjutsu to be him accepting himself. Once he initiated Izanami he started bringing up him accepting himself, with that being on Kabuto's mind he subconsciously made that the condition for leaving Izanami upon it being activated.

Conclusion
Thus Izanami is a 2 part jutsu. It not only requires the Caster to feel the same sensation twice in order to activate it but it also requires him to prep the enemy ninja to create conditions that would be favorable to the caster.
But it would be very hard to rile up someone that you have no intel on and refuses to talk not to mention that Izangi would be a better defensive jutsu while a genjutsu would be a better offensive jutsu. This jutsu is less of an offensive/defensive jutsu and more of a niche jutsu used to incapacitate an ninja ninja who is going through some inner conflicts while they come to grips with the truth.
This Scan says even more to the intelligence of Itachi. He says it is "already prepared", even though it really is not, there is alot left to be done. But by him saying that Kabuto lowers his guard or better yet will be looking for the wrong thing, he will be thinking that Itachi is using his words to distract him while he sets up his genjutsu when in reality Itachi is using the premise of the genjutsu in order to set him up with his words.
Its like the old saying about magicians. He had the bunny sitting in the false bottom of the hat the entire time, so when he waves his wand and tells you to pay attention the magic is already done. The key is misdirection.
