Tsuande is currently cut into two pieces so her chakra system around her body is destroyed and disrupted yet she could still summon Katsuyu. Therefore, Neji's inital attacks of him cutting off the initial tenketsu will not be of harm as she can still use chakra hence deliver a killer blow. This would be true for the initial cutting off of the tenketsu. Also if Tsunade pucnhes the ground, Neji will not be able to do the Hyuuga gesture before his moves as there will be a uneven ground.

Nothing to kill her with? Lets see...a palm strike to the heart is known to kill the opponent as demonstrated by Hiashi. I'm sure that neji could easily do the same to her. He could also seal all of her chakra points and kill her from a lack of chakra flow.

Anyways rotation can deal with any of her attacks and Neji could also use vacuum palm from a distance to knock her off balance so that he could rush in to strike.

Tsunade wont heal from blocked chakra points, and tsuandes strength becomes weaker with each chakra point blocked, so when Neji has a clear advantage here.

For that, she has to touch him, and against a master gentle fist user like Neji, I highly doubt the he is going to try to be touched in any way by tsunade when up close.

I feel like she could derange his pathways in the time it takes him to block like 6 chakra paths but that should be enough for her.
