Idk probably TB, cancer, aids. I would say TB because he was coughing up blood but anime people like to make coughing up blood not as big of a deal as it is in real life lol. TB is the only diesease I know of where you cough blood.

I mean if you look at Itachi's stats though. If he wasnt sick he would have the highest stats in all of the Naruverse

It can cause you to cough up blood actually, and can also cause you to have bloody urine (just to elaborate). Lung cancer and lymphomas are where you would typically see a person coughing up blood, but this would be seen in a very advanced stage of cancer.

Itachi was probably born with it, but whatever it was, Kishimoto knew that no character could have killed Itachi because of all the hype that has been given to him and because of what we've seen of him (never showed his real power) yet fodderized strong Shinobi in mere seconds. So Kishi nerved him with a disease instead and killed him that way for his plot surrounding Sasuke.

Like many things in Naruto, this was purposefully left open. Itachi was slowly destroying his eyes by having Mangekyo, and while Madara says Itachi was ill, Zetsu speculated that Itachi was actually injured, possibly from a battle he had right before meeting Sasuke ,possibly with Madara. But no definitive answers were given as of yet.
I think that his sickness has something to do with the Mangekyo but I dunno,we will have to wait and see...
