The Mist was absent from the third ninja war. Why was the mist absent? Because Obito was controlling Yagura the Mizukage at the time. The Land of Eddy's was coincidentally destroyed during that same general time frame. I assume Madara and Obito to have known and acknowledged the threat of the Uzumaki and their seals to hinder project Tsuki No Me. Obito achieved two things by doing this. The first being the near annihilation of the Uzumaki clan, and two, forcing out Nagato leaving him and his parents Hidden Rain bound. Finally, Obito could begin to project his influences on the young Nagato.

Technically speaking, no you actually don't have to kill the person closest to you, you have to experience intense feelings of loss to attain the Mangekyou Sharingan. Three-Tomoe Sharingan itself is still not to be underrated. With the Senju and Uzumaki, well, they're born with their strength. But only Hashirama and Tobirama are shown to be truly exceptional, and Tsunade is exceptional but definitely not all around. Naruto, well, most of his successes has been because of Kurama in all honesty.
