Am I the only one who noticed that she introduced herself to Isshin Kurosaki as Masaki Kurosaki?

I think he just took her name so that no one would recognize him. He is hiding from Soul Society probably, doesnt want anyone to find him, only people from SS who have seen him are all really young if anyone like Shunsui or Ukitake saw him they would probably recognize him

No her last name is Kurosaki...while Isshin's is Shiba.

I'm guessing Isshin took Masaki's last name so nobody would recognize the name as Isshin is originally from a noble house.

No, it's not a mistake.

Think about it, if Isshin indeed fled Soul Society like we all think, then it's the obvious thing to do to adopt a different name. Yes, he took Masaki's name. He probably also did this to conceal Ichigo's true heritage - the favored theory right now being from the Shiba Clan

As the latest chapter states, Isshin's name was Shiba. Of course you posted this thread before the latest chapter, but still even in that case it was perfectly explainable why Kurosaki was the name of Ichigo's mother.

When Ichigo popped up in SS, no one made, or in the least didn't show it, the connection between Ichigo and a previous captain of roughly 20 - 25 years ago. I mean suddenly an unknown shinigami comes along that has the same name as a relatively seen recent captain that disappeared (I doubt Isshin went away with the approval of SS). So yeah even without the recent chapter it would be a logical assumption that Ichigo took his mothers name to avoid troubles.
