We all know Hashirama was a brilliant Medical Ninjutsu User....

Madara had told Tsunade that even her Medical Ninjutsu Skills were no where near Hashirama's League....

It may be possible that Hashirama was also renowned for his Medical Ninjutsu and every person who was treated/operated by him was always saved except.... Izuna

Madara might have thought that Hashirama let Izuna die so that Madara would break and the Uchiha strength would suffer by all this....

When he might have shared his thoughts with the Uchiha Clan members they might have told him that he himself was responsible for taking Izuna's eyes and letting Izuna die....

So I think Hashirama's Failure to save Izuna let Madara Go The Wrong Way....

Moreover Izuna's death is still a mystery.... The basis of this theory is Hashirama being a great Medical Genius as said by Madara....
And Surely a good person like Hashirama would help others using his Medical Ninjutsu....
