Tobirama explained that for the Sharingan to be awaken, a Uchiha who had developed strong feelings on something/someone loses them, and thus profond despair from experiencing such loss must happen. If that happen, their brains release a special kind of chakra that form the Sharingan eye:
Does it mean that every Uchiha with Sharingan experienced that despair? If we think about it, Tobirama may be right. Remember that Sasuke for example awakened his Sharingan during the massacre probably because of the despair of losing his family that night. Concerning Itachi we don't really know but Tobi said he experienced many loses caused by the war, so it may be a reason
Probably the reason why not many Uchiha had Sharingan. Because they didn't experienced despair, and thus not having enough of hate(like Itachi said to Sasuke in part1)
