If there were many Uchiha who awakened MS, then why did the Hokages seem surprised that it wasn't the regular Sharingan Sasuke showed them? The second Hokage was shown thinking: "They're not the basic commas ... A Mangekyou Sharigan?"
That makes me assume that they actually hadn't encountered many Uchiha with MS before.

The chapter only stated that most Uchiha who awakened the Sharingan eventually turned evil, because they lost someone they loved. It didn't state that there were many Uchiha who awakened the MS.

I can see a grief striken crazed Uchiha spraying Amaterasu uncontrollably to be considered a threat and why the whole clan had a dubious distinction.

Maybe Itachi meant those four were the few elite ones that had control of thier MS and lived to use and master the nuances of the power(s). Those other MS Uchiha just don't count, as they were put down when their powers manifested for the safety of the village.

1. MS is very rare, and very less uchiha knows about the secret.
2. Itachi couldn't take down anyone else with MS plus other Uchihas with Sharingan at that age.
3. Manga said that only : Itachi, Sasuke, Madara, Izuna, Shisui, Obito have obtained Mangekyou ( Kakashi got Eye transplanted, his MS activated the same time as Obito, so I honestly think he got MS just because of Obito ) if not Kakashi also.
