Hashirama specificaly stated that he spoiled tsunade because she was his FIRST Grandaughter.
Does this matter? Is it pointless? Is it a mistranslation?

Also caught it. I am sure it is true if not a mistranslation. Other relatives could be dead. A lot happened in that time. I am sure it is just something that is an interesting tidbit but won't actually cause new plot.

Maybe, we'll have to wait and see if it gets revealed.

Technically, you can have a first grand-daughter without having a second. I'm not sure how the Japanese is used - it could be one of those sensitive contexts that would be implying "only" as opposed to "first."

It makes more sense for him to have spoiled his only grand-daughter than to have only spoiled his first. Though it is natural to have an extra-intense reaction to one's first experiences (regardless if another one follows them up).

often, when people refer to their grand kids.

they refer to em as the "first grand child" even though theres no second one yet in the anticipation that more would come.

i wanna know who's the mother and father though cause obviously that means hashirama fathered a child.

Probably not. Probably it says something like "She was the granddaughter of the first, so I spoiled her" type of thing.

I thought in some version he said "the first granddaughter"
not his first granddaughter.
Get it .

The first like the president and the first family
not first as in first and second
but if she did then it must be minato's parent.

I don't think it's important. Look at Asuma. He has another brother(konohamaru's dad) but we've never seen him. Just like how we'll probably never know who her parents were..jiraya's parents...minato's parents..etc. Familial questions are rarely answered unless dealing with the Uchiha.
