Imo Kabuto is a more serious concern. I'd put him only below Tsunade, maybe even above her.

He did this simply by pumping chakra into his system, accelerating cell division.

That requires immense chakra, but not necessarily knowledge or skill in the medical field.

He may have had such knowledge, though. We just don't know yet.
