Who in the Naruverse has the highest kill to death ratio? As in how many people they have killed compared to the amount of times they have died (orochi "died" like 2 times).

What about the ridiculous amount of lives Nagato brought back later? Katsu didn't save that many compared to the village population.

From what's been observed in the manga, it'd be Nagato.

He killed the vast majority of Konhagakure's residents and single-handedly ended Amegakure's previous regime.
He was reputed to have killed everyone ever associated with Hanzō (family, friends, and clients).
Nagato also has at least eight other confirmed kills and of course there's all the battles he's never lost. (Konan hype)

Sasori- Has a little less than 300 puppets and used his puppets to take over a nation

Madara- That giant meteor alone killed hundreds of people, not to mention in his clan battles in the past

Nagato- Destroyed the Leaf Village that held thousands of Ninja and civilians

Orochimaru- Experimented with live people, killed comrades to use as test subjects and killed people to attain immortality

Juubi- Before it was defeated and sealed it wreaked havoc in its time killing innocent people and ninjas as well

Death God- Claimed Billions of souls since the beginning of time xd
