Tsunade needs cells (old) to divide and make new ones, she can't do that if she's bombed on the cellular level.

No Tsunade will breathe since there is no reason for her not to. Tsunade won't know what happened since she is not a sensor and once she breathes just once, it is over.

She regenerate instantly so yes it will cost a lot of chakara though but she can handle it if she survie being rip to shreds with no regeneration then she can regenerate instantly from c4
