Giving them an illness is to buffer them from being too powerfull although they're already godlike and too strong for 98% of the characters in Naruto with their illnesses.

Kimimaro without the curse mark couldn't hold up against drunk Lee, and Nagato wasn't sick, he just lacked mobility which he could easily compensate with his summons.

Even in a weaken state nagato was tremendously OP, one of the strongest in the ninja world to the point that two ninja (obito and zetsu) atill didn't think he could lose, i can't say the same about itchi or kimimaro, they were never actually all that impressive, at least not to the level of nagato, and im not even a fanboy but im not blind to facts either.
