This is quite possible as we may never know. I would assume ems is always straight tomoes simply because two ms's as wildly different as mads and sasukes came out the same(as straight patterned) in ems. Although it would be kind of cool if there was another form of it that granted something else instead of more fluid movement

Yes, Itachi had God Tomoe a type of EMS surpassing every other Dojutsu

I was thinking it over and could it not have to do with the ems design, but ms design. Most of the ms designs we have seen have been spinning wheel designs. The only ones to not fit this pattern are sasuke and izuna. When madara got izuna eye he maintained his ms design, but itachi fit into the mold of sasuke's. I already believe that these ms designs have symbolic meanings to character, but maybe for some reason these straight ms designs carry enhanced abilities maybe even considered truer or more pure ms eyes. We know from sasuke's character even though he talked tough he still maintained his humanity and probably for the same for izuna.

When you transplant though you awaken a new eye pattern no? Maybe that's what he meant
