Go to a Dentist for Filling or Root Canal. Don't do what you are trying to do, you will regret it. Also pulling the tooth out is only logical if it's a Temporary/Milk Tooth. If you have a decaying Permanent Tooth, if you get it pulled out you will have a gap in your teeth. The best option then is to go for Root Canal and get a cap on it.

Have someone curb stomp you, real effective and will fix your problem quickly.

Don't. Your teeth are rooted and you can't properly take them out by yourself. You will end up breaking it leaving the root intact, or to hurt and start bleeding hard

For what purpose? I'm not sure what you are asking and if what I know is even relevant. I know more about Law than eye surgery you know.

I can say this much -it's mostly safe. I know a few people ( including family members) who got it done for cataract and a neighbor for to get rid of her glasses and it worked out for them.
