I would like say two of these counters you most certainly called out.

But, you have 2 and 4 mistaken.


Sasuke can see chakra.Unlike the byakugan however,he can't distinguish the chakra.
Zabuza could use a clone of any kind and all that would pop up is multiple sources of chakra.
Lastly,neji at age 13 who was clearly toying with naruto in the beginning is not someone that represented all hyuga or did that fight represent a fully determined to fight from the jump neji

Plus he was't even aware of the fox so he couldn't of possibly done anything on pupose to prevent somethign like that.

And if he did have that info pre fight he would of done exactly that since that is the kind of thing his kekegenkai excels at.

There were multiple times that Sasuke swaps himself with objects that are out of his field of vision

Such as when he evaded Madara's lightning
When he switched places with his sword
And When He Teleported Madara between himself and Naruto

Well in those scenes when he uses it, there was always a small panel of Sasuke's Rinnegan.
Also I dont think the jutsu activates at the center of his vision. Its just his field of vision overall.

I dont think Sasuke has 360 degree vision so if Sasuke was looking straight at Naruto I doubt Madara would have been in Sasuke's field of vision when Sasuke teleported him between himself and Naruto
