In theory, he could grab it. If Itachi's Susano'o can touch it so can Sasuke's, unless you're suggesting Sasuke can pass right through it? Anyways, if he does switch places, the Itachi has wasted chakra on making Susano'o and would have to erect a new one, in which case the same thing happens and he continuously wastes chakra until he runs out.

Susanoo can't defend against sound based Genjutsu and some other techniques like Hashirama's Wood release. Gaara also used his sand to pull Madara out of Susanoo by dragging him with his feet. He didn't directly pull him out through the center or front of the Susanoo. So I say Susanoo is enough. And I doubt Nagato would f*ck around with a technique with a 5 second rest interval against Itachi.
