Uchihas had been abusing Izanagi for ages, which is why Izanami was created - to counter its use. It's only the perfect form of Izanagi, which is 'The Creation of all Things' which requires what Obito stated. Two different things.

To use Izanagi at its full potential, one needs Hashirama's Cells.

With it the Izanagi lasts for a few seconds (see Danzo vs Sasuke).

Without it, it's a one shot. No seconds.

Yes he did. With his Mangekyou prowess, he used it in a similar manner Itachi did with Sasuke's eye.

It activates under certain conditions or time. He is Madara after all.
