Actually to be honest I can't remember if Madara had that face on his chest before he died and that's my whole theory to him getting Hashirma's flesh to awaken the Rinnegan by having a part of Hashirma's DNA and through him Ashura's. having both he had the requirements to awaken the Rinnegan but now I'm thinking that Kabuto added the face to Madara during resurrecting him to make him stronger so I'm kinda lost in my own thoughts...gonna go sleep now i think will pick this up tmr lol

The face is a result of fusing yourself with Hashirama's DNA. (Danzo had it as well.) So Madara had it prior to his death as well. (And as Sasuke's doesn't have it, he hasn't been fused with the DNA completely.)
